A Step-By-Step Guide/Checklist . (Please print this out and check each item off as you go)

• Please fill out our Medical Questionnaire and send it to us. We will give you a price quote and specific information about your procedure, along with available surgery dates.
• For a price quote, please send us photos of the areas you wish enhanced. We offer firm price quotes you can depend on by simply receiving a short medical history along with front and side photos of the areas to be improved. For privacy, you may take pictures of yourself through a mirror. You may also wear underwear or a two-piece swim suit. And for breast or body pictures, you may take pictures below the face if you wish. Please attach your photos to an email and send them to us at aceballos@guadalajaraplasticsurgerycenter.com
• Please check availability with the Guadalajara Medical Center Inn . Recovery time is required before you will be able to travel. Along with our price, we will give you the number of required days.
• Schedule a date for your surgery by giving us your preferred date here.
• You will need a valid Passport in order to travel to Guadalajara.
• Make an appointment for your pre-op tests. Please click here for the pre-surgery checklist and the tests you will need. You may print the checklist and take it with you when you go for your tests. Dr. Ceballos prefers the test results be done within 4 weeks of surgery. Blood tests and heart evaluation results should be as close to your surgery date as possible. You will also need an EKG if you are over 45 years of age. If you do not wish to have the tests done at home, you may have them done at our hospital at the time of your pre-op consultation at a minimum cost.
